To ensure accurate records and smooth access to care, we have recently resumed the practice of requesting health cards at each visit. You might remember we used to do this prior to COVID-19.
Even if you’ve been a patient for years, presenting your health card helps us:
Confirm your identity – We’re a large organization with more than 19,000 patients across two locations. Our staff are great at knowing patients/clients, but not every staff member knows every patient.
Ensure your information on file is accurate and up-to-date – You’d be surprised at how many times people forget to tell us about a move they made or that they discontinued their land line. You’d hate for your specialist referral to go to the wrong place, or miss the phone call with your important test results.
Maintain best practices for patient care – We strive to follow best practices and asking for health cards is one of them.
This small step helps us provide you with the best possible service and reduce errors in medical records.
In case your preferred name is different than your legal name on your card, that’s no problem. It’s easy for us to update your chart to reflect your preference. Just let reception or your provider know. If we don’t get it right, please tell us. We want to make sure we’re calling you by the correct name going forward.

The Occupational Therapy Department at Saskatoon Community Clinic is excited to announce an upcoming falls prevention group focusing on older adults who are at risk of falling in their home or caregivers interested in keeping their loved ones safe. This group will provide fun and interactive education on falls risk factors and prevention strategies, including easy changes that can be made to your lifestyle and your home.
Led by Occupational Therapists Rachel Degan and Julie Anstey, the group will run Tuesday, March 18 from 1:30 p.m. – 3:30 p.m. at our new location, 2211 11th Street West.
Snacks and refreshments provided.
